Fitness RX

Complimentary memberships are available through the FITNESS RX program. Crossover Health Services (CHS) prescribes fitness memberships (Fitness Rx) for established patients who want to take control of their health and would benefit from a safe place to exercise. As a patient of CHS, you are eligible for a complimentary membership based on the guidelines below or another condition determined by your provider. To enroll, meet with your CHS provider (CHS must be your primary care provider) and bring in the Fitness Rx referral form when you come to the Crossover Fitness Center.


Eligibility criteria includes but not limited to:
  • BMI over 35 kg/m2
  • Diabetes – most recent HbA1c over 8
  • Hypertension – most recent systolic 160+ or diastolic 110+

Sports Clinics
